Scholarship applications: a special form can be obtained from: Guidance Department Menominee High School 2101 Eighteenth Street Menominee, MI 49858 906-863-7814 In addition to the application form, a transcript of the three and half years of high school grades, two letters of recommendation, and an essay on the leadership qualities of the student are all required.
