How to ask

Individuals upon being referred by a physician to the Foundation for assistance with physical therapy services by an attending physician. Applicants will submit an application to the President and/or complete a personal interview to determine suitability for the program and lack of financial means. Grants small grants by other nonprofits may be solicited by letter or verbally at the Laurel Proof Foundation, Inc. board meetings.

Deadlines

There are no hard deadlines set. All applications received will be reviewed by the Board of Director

Limits on who can apply

The individual or organization applying for the funds must be either in financial need of the funds for physical therapy or they must desire to further study and develop mechanisms to aid those who are phsyically challenged.
