Applications for grants should be requested only in writing by organizations exempt under IRC 501 (c)(3). The request should include the organizations name, address, employer identification number, contact information, and a copy of its IRS determination letter.The proposed use of the funds, with supporting financial data, should be clearly defined as to amount and purpose.

Any awards are restricted to the purpose for which they were requested. An annual report of expenditures from any grant should be provided to the trustee no later than May 31 of each year.
